More than 7.6 million Americans suffering from acute illnesses or living with chronic health conditions receive in-home care, according to the U.S. Census Bureau. This number is even greater when you consider the number of people receiving informal care from family members, which may occur in up to 20% of U.S. households.
Most of these individuals receiving in-home care would otherwise be living in assisted living or other long-term care facilities when, in reality, they just want to stay in their own home. When families can't meet the needs of those who need care, they may choose to use various types of home care services.
Do you have Medical Assistance (MA)?
If yes, go to Step #2.
If no, you will need to apply for MA by contacting your county human services office or call the MN Care State Office at 651-297-3862 or 800-657-3672, or print an application at www.dhs.state.mnus/healthcare or apply online at www.applymn.dhs.mn.gov
Have you had a PCA services assessment and qualify for services?
If yes, go to Step #3.
If no, you will need to contact your county public health nurse or case manager to come out to authorize services. Choose Heartland PCA as your PCA provider at the time of the assessment.
