Agency Model: Consumer-Directed Community Support (CDCS)

Agency Model: Consumer-Directed Community Support (CDCS)

When you choose the “Agency Model,” Heartland PCA remains your provider. You are able to:
• Choose or design the services and supports that fit your assessed needs
• Decide when you should receive services and supports
• Hire the people you want (including a spouse, parent of minors, friend, family member, etc) to deliver services.

CDCS includes services and supports now available through Medical Assistance waivers. It also allows you to buy non-traditional supports, such as special therapies, assistive technology and home and vehicle modifications.

CDCS includes all services and supports available through Home and Community Based waivers. It also allows you to buy non-traditional supports that are not available through other waiver options.

Examples: Specialized therapies, special diets, adapted recreation, creative employment options

You can use CDCS if you are enrolled in one of these programs:

  • Developmental Disabilities (DD) Waiver
  • Community Alternatives for Disabled Individuals (CADI) Waiver
