Heartland is now an approved MN DHS Community First Services and Supports (CFSS) Agency Provider!

What is CFSS?
It’s Minnesota’s new program designed to replace personal care assistance (PCA) and the Consumer Support Grant (CSG), offering flexible, participant-centered support options. This means even more independence and tailored care for individuals in their homes and communities.

CFSS is available to those receiving PCA services through Medical Assistance (MA), an MA waiver, Alternative Care, MinnesotaCare, or a health plan—as well as those using the CSG.
