Individualized Home Support (With or Without Training)

This program is intended to provide youth and adults with disabilities the life skills to live independently in the community. Our direct care staff assist clients with services that develop, maintain and improve the community living skills. We do this by providing an assessment in the areas of need and developing measurable goals. Quarterly reviews are completed to do continued assessment of ongoing needs. Goals and objectives are modified as needed.

Training is direct from a staff member to address the identified skill development needs such as:

  • Communication skills
  • Community living and mobility
  • Interpersonal skills
  • Reduction/elimination of maladaptive behavior
  • Self-care
  • Sensory/motor development involved in acquiring functional skills
